If you've ever looked into body contouring, you've probably run into two names over and over: CoolSculpting and Emsculpt. They're often mentioned in the same breath, but they work in completely different ways. One is designed to destroy fat cells. The other builds real, working muscle.
At Chinn GYN, we chose not to offer cryolipolysis (the technology behind CoolSculpting). Instead, we offer Emsculpt, Emsculpt NEO and Emerald laser. That was a deliberate decision, and we want to walk you through our thought process.
Our goal is simple: to help you understand your options so you can choose what's right for your body.
How does CoolSculpting work?
CoolSculpting uses a process called cryolipolysis. A cooling applicator is placed on the skin to chill the fat underneath to a temperature that damages fat cells. Over the following weeks, the body clears away those damaged cells with the goal of having the treated area look slimmer.
It's FDA-cleared, widely available and noninvasive, which is why it became so popular. The approach is built on one idea, though: destroying fat cells in a specific spot.
Why destroying fat cells isn't the whole story
Fat often gets treated like something we simply want gone. In reality, fat tissue is a living, active organ. It's constantly communicating with the rest of your body through hormonal signals that affect metabolism, appetite and how your body stores energy.
When fat cells are destroyed rather than used, that signaling can be disrupted. Our providers have seen that results in the treated area may not last, and the body can compensate by increasing fat volume in other areas.
We look for treatments that work with your body's natural processes rather than against them.
What is paradoxical adipose hyperplasia?
You may have seen the term paradoxical adipose hyperplasia, or PAH, in the news. It's a rare but documented side effect of cryolipolysis. Instead of shrinking, the fat in the treated area grows larger and firmer, usually a few months after treatment. The medical literature reports a wide range of rates, roughly 0.025% to 1% of patients depending on the study (StatPearls, 2024). When it happens, it often needs surgical correction such as liposuction.
Most people who have CoolSculpting won't develop PAH. However, we share the risks because informed consent matters. These risks are a big part of why we prefer technologies that don't rely on freezing and destroying tissue.
How does Emsculpt work?
Emsculpt takes a different approach entirely. Instead of targeting fat, it targets muscle.
Emsculpt uses high-intensity focused electromagnetic energy to trigger powerful muscle contractions, far stronger and more frequent than you could achieve on your own in the gym. These are called supramaximal contractions. Your muscles respond the way they would to intense training: they adapt, grow and strengthen.
In studies, a series of four Emsculpt treatments has been shown to be the equivalent of about 16 weeks of intensive, daily high-intensity interval training (HIIT) for the treated area.
The muscle you build is real, and it's metabolically active.
Emsculpt Classic vs. Emsculpt NEO
We offer both versions, and the difference comes down to fat reduction.
-
Emsculpt Classic focuses on building muscle and improving muscle tone, with some added fat reduction.
-
Emsculpt NEO adds radiofrequency energy. This makes the contents of fat cells more available to your body as fuel, which supports even more muscle growth and more fat reduction in each session.
Your treatment specialist will help you decide which fits your goals and the area you want to treat, whether that's the abdomen, glutes, arms, thighs or calves.
Why muscle matters for long-term results
Building muscle doesn't just change how an area looks. Muscle tissue plays an important role in how your body regulates insulin and uses energy, and it influences where your body stores fat. In other words, the muscle you build with Emsculpt supports your overall metabolic health.
Those results are sustainable for most people. As long as you continue to engage and challenge the muscles you've built, you can maintain the gains.
We also want to be honest with you: Emsculpt is a passive treatment, so it doesn't raise your heart rate or give you the cardiovascular benefits of exercise. It isn't a substitute for movement. The metabolic benefit of the muscle you gain, however, is the same.
Where Emerald laser fits in
If your main goal is reducing fat, Emerald laser is another option we love. Emerald uses low-level laser energy to create a temporary opening in the fat cell, so it releases its contents. Your body then uses those contents as fuel and clears them naturally. The cell shrinks rather than being destroyed, which keeps your body's natural signaling intact.
That temporary opening stays available for up to 72 hours after each session, so eating a little lighter and moving a little more during that window can help you get the most from every treatment. Many patients pair Emerald with Emsculpt (the Melt ‘n Sculpt package) for both fat reduction and muscle tone.
So, which is better?
The right choice depends on your goals, your health and your body. Here's how we think about it:
| CoolSculpting | Emsculpt / Emsculpt NEO | Emerald Laser | |
|---|---|---|---|
| What it targets | Fat cells | Muscle (NEO also reduces fat) | Fat cells |
| How it works | Freezes and destroys fat cells | Electromagnetic energy builds muscle | Laser shrinks fat cells without destroying them |
| Works with your metabolism | No | Yes | Yes |
| Downtime | Minimal | None | None |
| Offered at Chinn GYN | No | Yes | Yes |
At Chinn GYN, we chose Emsculpt and Emerald because they align with how we approach all of women's health: support the body's natural processes, look at the whole person and focus on results that last.

Frequently asked questions
Is Emsculpt better than CoolSculpting? They do different things. CoolSculpting destroys fat cells with cold. Emsculpt builds muscle, and Emsculpt NEO also reduces fat. We prefer Emsculpt because it supports your metabolism and doesn't rely on destroying tissue.
How many Emsculpt sessions will I need? Most patients start with a series of four treatments, spaced about a week apart. Your provider will recommend a plan based on your goals.
Does Emsculpt hurt? Most patients describe it as an intense workout sensation. The intensity is adjusted to your comfort throughout the session.
What is paradoxical adipose hyperplasia? It's a rare side effect of cryolipolysis in which treated fat grows larger instead of shrinking. Reported rates range from about 0.025% to 1%.
Can I combine Emsculpt and Emerald laser? Yes. Many patients pair them for muscle tone and fat reduction with our Melt ‘n Sculpt package. Your provider can help you plan the right combination.
